Case no-3 of Covid-19 Case Series

Abstract: With the increased number of corona cases, many of the patients are now coming with more atypical symptoms without a fever. Atypical symptoms like Sore throat, Headache, Loss of smell or taste, Congestion or runny nose, nausea or vomiting, Diarrhea. Symptoms may present as a single or combination of symptoms. Therefore, cases of COVID‐19 with extrapulmonary and atypical presentations are not missed when they present to healthcare settings, where healthcare workers should remain vigilant, ensuring that appropriate personal protective equipment (PPE) is used.[1] It is important that healthcare professionals are aware of the possibility of COVID‐19 presenting with nonspecific symptoms. The present case presented as nausea, decreased appetite, weakness without fever and cough. The present case emphasizes the need to study the effect of homeopathic medicines in cases of corona presented with atypical symptoms.

Introduction: COVID-19 is a disease caused by a new strain of coronavirus. ‘CO’ stands for corona, ‘VI’ for virus, and ‘D’ for disease. Formerly, this disease was referred to as ‘2019 novel coronavirus’ or ‘2019-nCoV.’  It causes mild to severe respiratory illness. The virus has affected about 12 million people worldwide. 5,55,000 have died from this virus till now. With an increased number of cases, there are more cases presenting with atypical symptoms.

Risk factors: Around 1 in every 5 people who are infected with COVID-19 develop difficulty in breathing and require hospital care. People who are aged over 60 years, and people who have underlying medical conditions such as diabetes, heart disease, respiratory disease or hypertension are among those who are at greater risk. It’s important to diagnose even with atypical symptoms, as cases may still progress to have more typical symptoms and complications.

Symptoms: some of the atypical symptoms are:

  • COVID-19 exhibits a diverse range of clinical presentations. Whilst classical respiratory symptoms of a dry cough have been underscored, these may be preceded by atypical respiratory symptoms such as hemoptysis.
  • Additionally, progressive gastrointestinal symptoms (diarrhea, vomiting, abdominal pain) may be the index presentation, even occurring in the absence of other features.
  • Such symptoms have also been described in immunosuppressed patients, highlighting a potential at-risk group.
  • More recently, isolated anosmia or hyposmia has been widely reported as a primary symptom.
  • Ocular manifestations, primarily conjunctivitis, have also been pinpointed in case series, and the possibility of tear transmission has been broached
  • More generally, it is important not to neglect other disease manifestations since they may represent alternative modes of viral dissemination.[2]

According to a study the most common symptoms among people who were hospitalized with COVID-19 include: Fever, Fatigue, A dry cough, Body aches, Shortness of breath, Mucus or phlegm

Treatment: After repertorisation,

  1. Carbo vegetabalis 30, 4 doses were given at an interval of 1 hour.
  2. Patient slept after taking medicine and woke up with new energy, no nausea, no desire to take deep breath and could feel “there is oxygen in head”, it’s no longer heavy.

Plan: to give medicine on the basis acute totality.

Discussion: The case was treated as any other acute case, while considering the acute totality

In Materia Medica Homeopathic Medicines by S R Phatak, Carbo Vegatabalis

  • Nausea in the morning
  • Loathes even the thought of food
  • The simplest or smallest portion of food aggravating the sufferings of abdomen.
  • Eructation loud
  • Always weak, sick and exhausted
  • Cold sweat on forehead
  • Wants to take a deep breath

Conclusion: The physician should know that the corona cases can present only with atypical symptoms. The health worker should be careful about wearing PPE, lest getting himself infected. Like any other acute case, the homeopath should get acute totality of the case and prescribe according to that. The medicine should be repeated according to the severity.
